HASC’s Los Angeles Region includes facilities ranging in size from a nonprofit, university affiliated community teaching hospital of 1,258 beds to a sole provider community hospital of 16 beds — and a rural, critical access hospital of 12 beds. The county has 76 hospital emergency departments and 15 fully accredited trauma centers serving the almost 10 million residents.

The Orange County area office represents many types of health care facilities: general acute care, specialty pediatric, general acute care with behavioral health, acute rehabilitation, long-term acute care and an academic medical center. There is no county-owned hospital.

Orange County covers 789 square miles and has a population of over 3 million people. There are approximately 6,600 licensed hospital beds.

The SB 1152: Homeless Patient Discharge Planning Collaborative convenes hospital case managers and emergency department staff to assist them with the implementation of required activities for homeless patients as a result of Senate Bill 1152. In 2018-2019, the collaborative assisted with the development of a standard homeless patient discharge policy and education plan templates specifically for Inland Area hospitals. The collaborative checks-in on a quarterly basis to share their progress of implementation, challenges/barriers, and best practices.

SB 1152: Connecting All Platforms Workgroup – Inland Region

The Inland Region SB 1152: Connecting All Platforms Workgroup was established after the July 1, 2019, implementation of the homeless patient discharge log as required by Senate Bill 1152. The workgroup focuses on collecting and analyzing homeless patient data using multiple data platforms to develop a bridge between hospitals and appropriate providers and social services for homeless patients post-discharge. Currently, the workgroup is conducting a data collection pilot project at Riverside University Health System – Medical Center with an opportunity for the project to be replicated at other hospitals in the Inland Area.

Infectious Disease and Vaccine Guidance on Senate Bill 1152 for San Bernardino County
San Bernardino County Public Health Administration

Persons experiencing homelessness can face significant obstacles in accessing health services including basic health screenings and immunizations. SB 1152 requires hospitals to develop a written homeless patient discharge planning policy that coordinates services and referrals with regional social services agencies, medical care and county behavioral health services. This document is aimed at identifying specific communicable disease interventions and discharge planning strategies conforming to the requirements of SB 1152.

HASC, VCCF Partner on Caregiver Pilot Project

HASC, in partnership with the Ventura County Community Foundation, is supporting an innovative pilot project for caregiver support. Three hospitals are participating – Community Memorial Hospital, St. John’s Regional Medical Center and Adventist Health Simi Valley Hospital. VCCF has provided grant funds to supplement hospital dollars designated to this pilot.
