From May 2011 to February 2012, 20 Southern California hospitals participated in the inaugural session of the Case Management Transformation Initiative (CMTI), posting impressive first-round results. The initiative, a fee-for-service program offered by HASC in partnership with Marsh Risk Consulting’s Clinical Healthcare Consulting Practice, is data driven and provides participants with tools and strategies to transform case management programs into outcomes-based, physician-centric, case manager-as-care-coordinator programs.

The Case Management Transformation Initiative (CMTI), powered by Marsh, is a nine-month educational program that combines Marsh’s case management consulting practice with HASC’s proven collaborative model of performance improvement. Together, we provide hospitals with in-person support, education and networking that leads to accelerated change.

How It Works

Each nine-month program is limited to 20 hospitals. Participating hospitals create four-member teams that attend live, monthly collaborative sessions to facilitate learning and adoption of best practices. Teams review baseline and peer data cost analysis in the kick-off meeting which is followed by educational web conferences, on-site consulting, access to metric and dashboards, and skills assessments to measure each hospital’s progress.
