Rapid Applications for Medical Programs (RAMP) is a customized
web-based tool used to screen for financial assistance programs
like Medi-Cal, CMSP, Crime Victim Compensation, Section 1011 and
hospital assistance/charity.
First introduced to HASC members last year, RAMP is already
providing a significant return. Centinela Hospital Medical Center
began RAMP implementation March 15, 2012. Within 60 days, the
hospital screened 170 patients.
There is still time to participate in HASC’s Case Management
Transformation Initiative (CMTI), scheduled to begin in October.
Powered by Marsh, CMTI is an interactive nine-month,
results-oriented transformation program that will provide your
hospital with the tools to transform your current case management
function into a comprehensive, physician-centric, outcomes-based
program.
The Bridges Maternal Child Health Network (MCHN), under the
management of Orangewood Children’s Foundation (OCF) and funding
from the Children and Families Commission of Orange County
(CFCOC), provides a multi-disciplinary system of care to improve
the health and well-being of pregnant women and young children
0-5 years of age throughout Orange County. MCHN recently released
its Fiscal Year 2011-12 report.

participants with tools and strategies to transform case
management programs into outcomes-based, physician-centric, case
manager-as-care-coordinator programs.

How It Works
Each nine-month program is limited to 20 hospitals. Participating
hospitals create four-member teams that attend live, monthly
collaborative sessions to facilitate learning and adoption of
best practices. Teams review baseline and peer data cost analysis
in the kick-off meeting which is followed by educational web
conferences, on-site consulting, access to metric and dashboards,
and skills assessments to measure each hospital’s progress.

Working with patients along the continuum of care is increasingly

help managing their finances or other personal needs after
discharge, the Conservatorship Access Network (CAN) offers
comprehensive probate conservatorship evaluation at your
