A collaborative model for performance improvement
Develop a results-based case management program that engages physicians and case managers for more effective care coordination. Join the Case Management Transformation Initiative (CMTI).
The Case Management Transformation Initiative (CMTI) powered by Marsh is a nine-month educational program that combines Marsh’s case managemenrt consulting practice with HASC’s proven collaborative model of performance improvement. Together, we provide hospitals with in-person support, education and networking that leads to accelerated change.

How It Works
Each nine-month program is limited to 20 hospitals. Participating hospitals create four-member teams that attend live, monthly collaborative sessions to facilitate learning and adoption of best practices. Teams review baseline and peer data cost analysis in the kick-off meeting which is followed by educational web conferences, on-site consulting, access to metric and dashboards, and skills assessments to measure each hospital’s progress.

Readmissions Reduction Collaborative (RRC)

The Readmissions Reduction Collaborative (RRC) is an 18-month program that reduces the number of preventable readmissions for high-risk patients.
The Hospital Association of Southern California (HASC) and the Society of Hospital Medicine (SHM) modeled the new fee-for-service program on Better Outcomes for Older Adults through Safe Transitions (Project BOOST), created by SHM in 2008. RRC aims to broaden the scope of that project to meet the needs of qualified L.A. Care patients and providers, improving quality and outcomes across the continuum of care.

The Program — Team Strategies and Tools to Enhance Performance and Patient Safety designed by the Agency for Healthcare Research and Quality and the Department of Defense — is much more than a single training session or package. TeamSTEPPS teaches healthcare professionals how to integrate teamwork principles into daily practice throughout the organization.
