As we approach the new era of health care reform, hospitals and
health systems will need to create more efficient, innovative
ways to deliver health care. This will have a substantial impact
upon organizations’ human capital. HR leaders must seriously
examine the engagement culture in their organizations and
consider revising or developing new strategies to better prepare
for the impact of health care reform.
Dr. Eric Coleman, Director of the Colorado Care Transitions
Program, will describe a seven‐prong approach to improving
quality and safety during care transitions. He will explain
that there are “quick fixes” for this aspect of health care
delivery, then present a multifaceted approach that has
proven to be effective and include the key steps to
operationalize true patient-centered care along the
continuum.
