Person-Centered Care Collaborative

Person-centered care is an emerging health care paradigm that seeks to tailor care to match the individual needs and preferences of older adults.

This three year initiative will facilitate the testing and implementation of various models of transitional and continuum care in 3-4 provider groups of hospitals, post-acute providers, and community base partners, who will work collaboratively to address person-centered social determinants of health and patient/family activation, as well as clinical disease management.
